Co-Occurring Mental Health and Substance Use in Teens and Young Adults: Things Families Should Know

When a young person is struggling, it is rarely just one thing. A teenager who is withdrawing from friends, failing classes and using substances may also be dealing with anxiety, depression, trauma or a condition that has gone unnoticed for years. Professionals call this combination co-occurring disorders, and it is far more common than many families realize. The challenge is that substance use and mental health symptoms can look so alike, and feed each other so strongly, that it can be hard to know where one ends and the other begins. Understanding how they connect can help families seek the right kind of care and avoid common pitfalls. Here are seven things every family should know.

  1. What Co-Occurring Really Means

Co-occurring simply means that a person has both a mental health condition and a substance use concern at the same time. The mental health side might include anxiety, depression, ADHD, mood disorders or trauma-related symptoms. The substance side might involve alcohol, cannabis, vaping, prescription pills or other drugs.

Jessica Morgan, Clinical Program Director at New Harbor Behavioral Health, explains that young people often reach for substances as a way to cope with feelings they do not know how to name. She emphasizes that this does not mean the substance use is harmless, only that it is usually trying to solve a problem. When families understand that a teen’s behavior has a purpose, they can respond with curiosity and compassion rather than only punishment.

  1. Why the Two Feed Each Other

The relationship between mental health and substance use runs in both directions. A teen with anxiety may use substances to relax or fit in socially, and then find that anxiety rebounds more strongly once the effects wear off. A teen with depression may use to escape low mood, only to find that regular use deepens it. Over time, a loop forms that is hard to break without help.

This is also why simply taking away the substance rarely resolves things on its own. If the underlying distress remains, the pull to cope in the old way remains too. Effective care looks at the whole cycle, not one piece of it.

  1. Warning Signs That Deserve Attention

No single sign is definitive, but a pattern of changes can be a signal. Parents may notice shifts in sleep, appetite or energy, a drop in school performance, withdrawal from family or longtime friends, new secrecy, sudden mood swings, or a loss of interest in activities that used to matter. Physical clues such as red eyes, unusual smells or missing money can add to the picture.

It is important not to wait for certainty before acting. Early conversations with a pediatrician, school counselor or mental health professional can clarify what is happening and open the door to support before things escalate. Trust your observations, and write down specific examples so you can describe them clearly.

  1. Why Treating Only One Side Falls Short

If a teen receives help for substance use but their anxiety or depression is never addressed, relapse risk can rise because the original struggle is still there. Likewise, treating a mood or anxiety condition while ignoring ongoing substance use can limit progress, since substances can interfere with therapy, sleep and medication effects.

Integrated care, where both areas are assessed and treated together by a coordinated team, tends to make more sense. It helps young people learn healthier coping tools, build emotional awareness and understand how their thoughts, feelings and choices connect. Families should feel comfortable asking any provider how they handle co-occurring conditions specifically.

  1. The Family’s Role Is Bigger Than It Feels

Parents and caregivers often feel sidelined once treatment begins, but they are central to progress. Learning about the conditions involved, practicing supportive communication and taking part in family sessions can change how the whole household functions. It also helps parents manage their own stress, which is real and valid.

Practical steps matter too. Keeping a consistent routine at home, securing alcohol and medications, setting clear and fair expectations, and acknowledging effort rather than only outcomes all help create an environment where healing is possible. Shame and blame tend to push young people away, while connection draws them in.

  1. Life After Treatment Needs Structure

Leaving a treatment program can be both exciting and frightening. The structure and support that were part of daily life suddenly shift, and a young person has to apply new skills in the same environment where old patterns developed. Without a plan, it is easy to slide back into familiar habits.

A good aftercare plan usually includes continued therapy, regular check-ins, healthy routines for sleep, school or work, and a clear understanding of triggers and how to respond to them. It should also include a plan for what to do if things start slipping, so that a stumble does not turn into a crisis. Having this conversation before leaving treatment helps everyone feel more prepared.

  1. Transitional Living Can Bridge the Gap

For some older teens and young adults, going straight from a treatment setting back to full independence can be too big a leap. Transitional or sober living environments provide a middle step, with structure, accountability and peer support while the person practices daily life skills such as holding a job, managing time and maintaining relationships.

Amanda Foster, Director of Recovery Services at Riviera Recovery, says that people often do best when they have a supportive community around them during the months after formal treatment. She notes that building routines, such as consistent work or school schedules, healthy habits and regular meetings, gives young adults real-world confidence, and that they are more likely to keep those habits when they have chosen them for themselves. For families, knowing this option exists can ease the fear that a young person will be left alone at the most fragile moment.

Conclusion: Hope Is Realistic

Co-occurring mental health and substance use challenges can feel overwhelming, but they are treatable. Young people can and do learn healthier ways to cope, rebuild relationships and move toward goals they care about. The path is rarely perfectly smooth, yet each step, whether it is an honest conversation, a first therapy session or a month of steady routines, matters.

If you suspect a teen or young adult in your life is struggling, start the conversation, seek an assessment and look for care that treats the whole person. Reaching out early is one of the most helpful things a family can do, and it signals to the young person that they are not facing any of this alone.
